Add/Drop Form
Accuplacer CPT Score Sheet - Shows how the raw score from the placement test correlates to placement into classes.
Application for Transfer of Credit - Submitted with official college transcripts, Advanced Placement Scores from College Board, high school transcripts for intermediate algebra proficiency, and military transcripts.

Path to English 101 with conversion from old numbering system
Excess Credit Request - Used when a student wants to take over 18 credits. They must have completed 30 college level credits and have a 3.0 GPA or higher.
Grade Renewal - Eliminate credits and grade(s) from computation of GPA for courses that I have repeated satisfactorily at South Puget Sound Community College.The lower grade will not be calculated into the GPA. All grades will continue to appear on the transcript. An “R” will appear next to the lower grade that won’t be calculated into the GPA.
Graduation Application - Student are asked to submit their graduation application to Enrollment Services 2 quarters prior to expected graduation date. This is done online!

Special Enrollment Request - Used for course susubstitutions and waive rs. Form is turned into Enrollment Services and becomes part of the students graduation file.
